Program Overview
Sometimes parents need help. If your child is struggling with a mental health issue, educational success, a developmental delay/disability, an alcohol or drug problem, or a combination of these things you may qualify for a Child and Family Team (CFT). Sharing your family’s story may be difficult, especially if you are sharing it with strangers or having to tell it multiple times. The Child and Family Team wraparound program is individualized and strength-based. The program works to empower parents and youth to be strong advocates for their needs.

- Champaign County Public Schools – public education entities (Graham, Mechanicsburg Exempted Village, Triad Local, Urbana City, and West-Liberty Salem)
- Champaign County Board Developmental Disabilities – supporting individuals with developmental disabilities
- Champaign County Department of Job and Family Services – public assistance, child welfare, employment, and child support
- Champaign County Domestic Relations-Juvenile-Probate (Family) Court
- Champaign County Early Intervention/Help Me Grow – early intervention and home visitation for children 0-3

- Parent Mentor – Jacqueline Howley – assistance with understanding the special education process and parental rights within special education

- Wellspring – mental health provider, special focus on early childhood
How to make a referral
All Champaign County families who wish to participate in a CFT must complete a referral packet (link to packet below) or contact our office for assistance with completion of referral paperwork. In addition, all Champaign County social service agencies, organizations, schools, and the juvenile court who work with children have the ability to make a referral to the CFT program.
The child(ren) should meet the following criteria:
- Custodial parent must be a resident of Champaign County.
- The child(ren) must not be over the age of 21 years.
- The child(ren) must have multiple-system unmet needs.
While completing a referral, the parent, custodian, or guardian must agree to participate in the CFT program and sign the CFT Universal Release of Confidential Information form. Participation in the CFT program is voluntary and parents have the right to opt out of services at any time.
